Transition Coordinator
We are a non-profit, non-residential Center for Independent Living providing services for people with disabilities and people feeling the effects of aging.
Work to assist individuals from institutions to the community of their choice utilizing appropriate community based services. Must have knowledge of services utilized by people with disabilities who successfully live on their own. Bachelor’s degree in social work or a related human service area of study preferred. Good organizational, planning, communication and problem solving skills preferred. Must be able to travel within a 14 county NE ND service area frequently; throughout the State less often.
